Home Health Physical Therapist

Provides services to home care clients in accordance with the Plan of Care. Services are provided under the direction of the attending physician with participation of the family and other members of the interdisciplinary team, as indicated.

Salary: $38.70 — $57.24 / hour

Required Qualifications:

  • Graduate of bachelor's or master's degree in physical therapy from an accredited program

  • Current and unrestricted license as a Physical Therapist in the state of Alaska

  • Minimum of one (1) year experience in an acute care setting or equivalent experience

  • Current BLS/CPR certification

  • Strong communication and interpersonal skills

  • Valid driver’s license and car insurance

  • Reliable transportation

  • Willing to travel from home to home, participate in on-call and weekend/holiday coverage rotations 

  • Good driving record

  • Knowledge of a variety of physical therapy techniques and modalities

Essential Functions:

A. Performs physical therapy assessments, diagnostic tests, skilled treatments, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care

 

  • Completes assessments in a timely manner and in accordance with Agency policy

  • Develops a treatment program and establishes goals for improved function

  • Provides assessment findings and goals to assist in developing the client care plan and obtains specific physician orders as needed

  • Documents all findings, plans, interventions, and client progress (outcomes) on designated forms and submits per Agency guidelines

  • Communicates plans and changes to the physician and to nursing Case Manager and other Agency staff through the care plan, written progress notes, and participation in care conferences

 

B. Educates patients and their families on how to manage their conditions and prevent future injuries

 

C. Advises, consults, and teaches appropriate treatment procedures and supportive activities to clients, family members, and other Agency staff

D. Designs and/or orders appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning

 

E. Instructs clients and their families in the client specific program

 

F. Performs job in compliance with Agency policies and procedures and professional and community standards 

 

  • Provides services in accordance with applicable state, federal and The Joint Commission standards

  • Attends meetings and required educational programs

  • Accepts responsibility for personal and professional development and identifies learning/developmental needs to supervisor

 

G. Attends care conferences, initiates interdisciplinary referrals and participates in care coordination as needed

 

H. Performs other related duties and responsibilities as deemed necessary

Salary: The hourly range for Alaskan residents is based on the prevailing wage rates issued by National Prevailing Wage Center for the current year, as well as  several factors including but not limited to education, work experience, certifications, etc.
